Copays definition

Copays or "Copay" - a dollar amount that You must pay directly to a Provider for certain Covered Health Services. Such dollar amount is in addition to the Premium. (See also Coinsurance.) "Cosmetic Procedures" - procedures that improve physical appearance but do not correct or materially improve a physical function. Cosmetic Procedures include, but are not limited to, drug treatment, plastic surgery, and nutritional procedures and treatments. "Cover" - pay for Health Services to the extent they are Covered under this Contract.
Copays. If a copay is required by the patient’s insurance, it will be due at the time of service.
Copays means any applicable co-insurances, co-payments, copays and/or deductibles.

Examples of Copays in a sentence

  • These include, but are not limited to, premiums, Copays, Deductibles and Coinsurance.

  • HNE will pay you for the cost of Covered Services, less any applicable Deductible and Copays or Coinsurance.

  • Any health care expense, including Coinsurance, or Copays and without reduction for any applicable Deductible that is covered in full or in part by any of the Plans covering the Insured Person.

  • Covered Services from Plan Providers are paid at the In-Plan level and are covered in full, except for any Deductibles, Copays and Reductions of Benefit listed in this EOC.

  • Allowable Expenses: Any health care expense, including Coinsurance, or Copays and without reduction for any applicable Deductible that is covered in full or in part by any of the Plans covering the Insured Person.

  • Any applicable Coinsurance, Copays, or Deductibles will be applied to the Out-of-Pocket Maximum.

  • All Plan Copays, Deductibles, maximums, limitations and exclusions will apply.

  • Copays, Premiums, balance- billed charges, and health care this plan doesn’t cover.

  • Copays, Coinsurance, and Reductions of Benefits are listed in the Summary of Benefit Chart in Appendix A.

  • Co-pays and/or deductibles, set forth by the OHC, cannot be collected from a Medicaid recipient for a Medicaid covered service.

Related to Copays

  • Copayment means that you pay a fixed amount each time you fill a prescription.

  • Medical Expense means an expense incurred at the time a past member or his or her health reimbursement account dependent is furnished the medical care or service. To be considered a medical expense under this act, the expense shall meet all of the following conditions:

  • Health Plans means any and all individual and family health and hospitalization insurance and/or self-insurance plans, medical reimbursement plans, prescription drug plans, dental plans and other health and/or wellness plans.

  • Adult education means all education or instruction,

  • Medical physicist means a person trained in evaluating the performance of mammography equipment and facility quality assurance programs and who meets the qualifications for a medical physicist set forth in 41.6(3)“c.”

  • Medical Specialist means any medical practitioner who is vocationally registered by the Medical Council under the Health Practitioners Competence Assurance Act 2003 in one of the approved branches of medicine and who is employed in either that branch of medicine or in a similar capacity with minimal oversight.

  • Indigent means a person who is not financially able to employ counsel.

  • Health insurance carrier or "carrier" means any entity subject to the insurance

  • Medical Expenses means those expenses that an Insured Person has necessarily and actually incurred for medical treatment on account of Illness or Accident on the advice of a Medical Practitioner, as long as these are no more than would have been payable if the Insured Person had not been insured and no more than other hospitals or doctors in the same locality would have charged for the same medical treatment.

  • Adolescent means a Medicaid patient 17 years or younger.

  • Outpatient means a person who is receiving care other than on an inpatient basis, such as: • in a provider’s office; • in an ambulatory surgical center or facility; • in an emergency room; or • in a clinic. .

  • Medically necessary care means care that is (1) appropriate and consistent with and essential for the prevention, diagnosis, or treatment of a Patient’s condition; (2) the most appropriate supply or level of service for the Patient’s condition that can be provided safely; (3) not provided primarily

  • Health plan or "health benefit plan" means any policy,

  • Travel Expenses means any costs incurred by Licensor associated with the transportation, storage or lodging of equipment, supplies, Licensor employees and other items necessary for business use from Licensor headquarters to Licensee’s facilities. Travel expenses may include, but are not limited to airfare, hotel costs, and meals if applicable. Any travel expenses paid by the Licensee shall be paid at allowable government travel rates consistent with Management Directive 230.10, unless otherwise first approved by the Licensee’s authorized representative.

  • Local anesthesia means the elimination of sensation, especially pain, in one part of the body by the topical application or regional injection of a drug.

  • Medical Reimbursement Programs means a collective reference to the Medicare, Medicaid and TRICARE programs and any other health care program operated by or financed in whole or in part by any foreign or domestic federal, state or local government.

  • Medicare eligible expenses means expenses of the kinds covered by Medicare Parts A and B, to the extent recognized as reasonable and medically necessary by Medicare.

  • Preschool child means a child who is three years old or older but is not a school child.

  • Midwife means a midwife in good standing with the College of Midwives of Ontario;

  • Medical assistance means medical assistance as established under title XIX of the social security act, 42 USC 1396 to 1396w-5.

  • Child or covered child means a child (natural, step-child, legally adopted or a minor or disabled child who is dependent on the insured for support and maintenance) who is born to or brought to a marriage or to a civil union established according to Vermont law.

  • Health insurance means protection which provides payment of benefits for covered sickness or injury.

  • Participating Marriage and Family Therapist means a Marriage and Family Therapist who has a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.

  • Nursing home-type patients means a patient who has been in hospital more than 35 days, no longer requires acute hospital care, cannot live independently at home or be looked after at home, and either cannot be placed in a nursing home or a nursing home place is not available.

  • Medically Necessary means a service which is appropriate and consistent with the treatment of the condition in accordance with accepted standards of community practice.
