All Client Authorization for ROI forms related to the HMIS will be placed in a file to be located on premises and will be made available to HAWNY for periodic audits.
Agency will keep signed copies of the Client Authorization to Disclose Information for a period of seven (7) years after expiration.
Once the Client Authorization to Disclose Information expires, the Agency will no longer edit or print the record and any new information entered into HMIS will not be accessible to other participating agencies.
With a current Client Authorization to Disclose Information form on file, Agency may update, edit, and print out a client’s information.
Subject to the above Client Authorization to Disclose Information requirements and the parameters set forth in Appendix A, Agency’s client information will be shared with other participating agencies, or programs within a participating agency.
Client may add additional Users beyond the authorized number at any time in accordance with the rate per additional user agreed to on the Client Authorization Form, or by execution of a mutually agreeable written amendment to the Authorization Form.
The Agency acknowledges that an adapted Agency Authorization to Release Information form must include an HMIS clause which permits the Agency to share restricted client information with select agencies (as specified on the Client Authorization Form) in compliance with the Agency’s approved Confidentiality Policies and Procedures.
Sharing of restricted information is not covered under the general HMIS Client Authorization Form.
The Agency agrees to place all Client Authorization forms related to the HMIS in a file to be located at the Agency's business address and that such forms will be made available to the HLA for periodic audits.
The Agency agrees to place all Client Authorization for Release of Information forms related to the MSHMIS in a file to be located at the Agency's business address and that such forms will be made available to the MCAH for periodic audits.
