Cadillac Plan. Taxes: the federal government plans to impose a tax on any health plan an employer offers that has a total value greater than $10,200 for single coverage or $27,500 for family coverage; the premium thresholds for such high value health plans may be modified from time to time by the federal government. To the extent the premiums for any medical plan offered by the Company exceed the government-mandated thresholds and that Plan is subject to this Cadillac Plan tax, employees who enroll in the Plan will be responsible for payments to the Company of amounts equal to the Cadillac Plan tax. These payments will be added to the employee’s premium contributions.

To the extent the premiums for any negotiated medical plan offered by the Company exceed the government-mandated thresholds and that Plan is subject to this Cadillac Plan tax, Frontier will consult with its actuaries and make design changes necessary to ensure the total value of the plan remains below the government-mandated Cadillac Plan tax thresholds for all levels of the overage (but only to the extent necessary to ensure the plan is not subject to the Cadillac Plan tax).

  • Flexi Plan means any individual indemnity hospital insurance plan under the VHIS framework with enhancement(s) to any or all of the protections or terms and benefits that the Standard Plan provides to the Policy Holder and the Insured Person, subject to certification by the Government.   • Nursing program means a series of courses leading to an LPN certificate or an ADN degree provided through a Wyoming community college; or a BSN degree or a Master’s degree provided through the University of Wyoming; or, in the case of a candidate for a doctoral degree, provided through an institution offering the required nursing education courses under contract with the WICHE, or in a distance nursing education program provided through a university that is regionally accredited.

  • Forest management plan means a written plan prepared and signed by a qualified forester that prescribes measures to optimize production, utilization, regeneration, and harvest of timber. The forest management plan shall include a schedule and timetables for the various silvicultural practices used on forestlands, which shall be a maximum of 20 years in length. A forest management plan shall include all of the following:

  • Asset Management Plan means a plan created by the department and approved by the state transportation commission or a plan created by a local road agency and approved by the local road agency's governing body that includes provisions for asset inventory, performance goals, risk of failure analysis, anticipated revenues and expenses, performance outcomes, and coordination with other infrastructure owners.

  • Nutrient management plan means a plan developed or approved by the Department of Conservation and Recreation that requires proper storage, treatment and management of poultry waste, including dry litter, and limits accumulation of excess nutrients in soils and leaching or discharge of nutrients into state waters.

  • Integrated Development Plan means a plan formulated and approved as envisaged in Section 25 of the Municipal Systems Act 2000, as amended.

  • Medical cannabis card means the same as that term is defined in Section 26-61a-102.

  • Medical flexible spending arrangement means a benefit plan

  • North American Numbering Plan (NANP) means the numbering architecture in which every station in the NANP Area is identified by a unique ten (10)-digit address consisting of a three (3)-digit NPA code, a three (3)-digit central office code of the form NXX, and a four (4)-digit line number of the form XXXX.

  • Transition Plan means a transition plan, acceptable to the LHIN that indicates how the needs of the HSP’s clients will be met following the termination of this Agreement and how the transition of the clients to new service providers will be effected in a timely manner; and

  • Parent Plan has the meaning set forth in Section 7.8(b).

  • Non-Administrator Ambulatory Surgical Facility means an Ambulatory Surgical Facility which does not meet the definition of an Administrator Ambulatory Surgical Facility.

  • HACCP plan means a written document that delineates the formal procedures for following the hazard analysis and critical control point principles developed by the National Advisory Committee on Microbiological Criteria for Foods.

  • Health plan or "health benefit plan" means any policy,
