Associated Provider definition

Associated Provider means the Healthcare Providers approved by Us to provide Treatment for which a Benefit may be payable. Benefit(s) means the insurance cover provided by this Policy and any extensions or restrictions as shown in the Schedule of Benefits or in any endorsements (if applicable). Child means a Principal Member’s own son or daughter, stepson or stepdaughter, or any dependant minor who is legally adopted who is in the Principal Member’s custody. Chronic means a Medical Condition which has at least one of the following characteristics: • it continues indefinitely and has no known cure. • it comes back or is likely to come back. • it is permanent. • Its signs and symptoms have been present for the last 3 months. and requires long term monitoring, consultations, checkups, examinations or tests by a Co-payment means the amount that the Member will be required to contribute towards the cost of a Medical Practitioner’s consultation fees for each consultation. Date of Entry means the date shown on the Schedule of Insured Persons on which a member was included under this Policy. Day Surgery/ Procedure means any procedure performed as Outpatient but would ordinarily be performed as an In Patient. Healthcare Provider seek authorisation from Us prior to any Day Case Surgery. Dependant(s) means one spouse and/or unmarried children who are not more than 21 years and residing with the Principal Member or 25 years old if in full-time education. Proof of continuous/on-going education should be attached. Dependants Application Form mean the form that Dependants must sign to apply for cover under this Policy, including any written statement, representation or document given to Us which contains information We relied on in issuing this Policy.
Associated Provider means any company which at the relevant time is (i) a holding company of the Provider; or (ii) a subsidiary or subsidiary undertaking of the Provider; or (iii) a subsidiary or subsidiary undertaking (other than the Provider itself) of any such holding company, the expressions “holding company”, “subsidiary” and “subsidiary undertaking” shall have the same meaning as is given to them by section 1162 of the Companies Act 2006 (as amended). “Audit” means an audit carried out pursuant to Rule 21 of Part Three of the Rules "Auditor" means the National Audit Office or an auditor appointed by the Audit Commission and/or the European Commission and/or European Court of Auditors (as applicable) (or in each case, an auditor appointed by the relevant body) as the context requires “Best Practice” means SDS recommended format for documents to be used by the Provider to record information required by the terms of the Provider Contract (available on the SDS website) “Bribery Act” means the Bribery Act 2010 and any subordinate legislation made under that Act from time to time together with any guidance or codes of practice issued by the relevant government department concerning the legislation “Change of Circumstance Form” means form set out in Appendix 6 notifying JCP of a change in a Participant’s circumstances “Change of Control” has the meaning set out in Rule 14.2 of Part Three of the Rules “Contract Schedule” means the contract entered into between SDS and the Provider when SDS requests services from the Provider “Contract Term” means the period from 1 April 2016 until 31 March 2017Controlling Interest” means:
Associated Provider means a provider that enters into an arrangement with a listed awarding body under which arrangement the provider provides a programme of education and training that satisfies all or part of the prerequisites for an award of the listed awarding body which is included in the Framework;

Examples of Associated Provider in a sentence

  • Some Healthcare Providers are NOT on Our Associated Provider List and will not have agreements with Us. If a member receives medical Treatment or services from one of these Healthcare Providers, the Member will be required to settle the charges for the Treatment or services rendered and submit the invoices to Us for reimbursement.

  • We would prefer that Treatment is carried out by a Healthcare Provider on Our Associated Provider List.

  • Network represents and warrants that such Network Amendment is deemed approved by and legally enforceable against such Network Provider in accordance with the terms herein and in accordance with its terms, except as such Network Associated Provider provides effective notice to Network within 20 days of receiving such Network Amendment that such Network Associated Provider does not accept such Network Amendment.

  • We will settle claims directly with Healthcare Providers and Medical Practitioners who are on Our Associated Provider List.

  • Such Network Associated Providers who provide such notice rejecting such Network Amendment shall, for the purposes of this Agreement, not be a Network Provider, unless such Network Associated Provider executes a Three Party Agreement (as defined below).

  • Claims for medical Treatment incurred in Rwanda are acceptable for assessment and payment direct to the Healthcare Provider, if the Medical Practitioner concerned is on Our Associated Provider List.

Related to Associated Provider

  • Affiliated provider means any provider that is employed by or has entered into a contractual

  • Qualified provider means a person or business experienced in the design, implementation, and installation of energy conservation measures. A qualified provider to whom the contract is awarded shall give a sufficient bond to the school district for its faithful performance.

  • Approved provider means a provider of services registered with the appropriate association or organization to render services to a member and approved by Latrobe to render services to its members.

  • Registered Provider means a registered provider of social housing (see section 80 of the Housing and Regeneration Act 2008);

  • Related provider trust means a titling trust or other trust established by a licensed viatical settlement provider or a financing entity for the sole purpose of holding the ownership or beneficial interest in purchased policies in connection with a financing transaction. The trust shall have a written agreement with the licensed viatical settlement provider under which the licensed viatical settlement provider is responsible for ensuring compliance with all statutory and regulatory requirements and under which the trust agrees to make all records and files related to viatical settlement transactions available to the commissioner as if those records and files were maintained directly by the licensed viatical settlement provider.
