Accident Reports definition

Accident Reports are defined in subsections 4.2 through 4.4.
Accident Reports. You will notify us immediately if the product is lost, stolen, damaged, destroyed, or if any person is injured or dies or if any property is damaged as a result of use of the product. You agree to file all reports as required by law, as well as those required by applicable insurers. You are solely responsible for completing and filing any accident reports as required by law. In the instance of a claim, You, Your agents and Your employees will cooperate with us and all insurers providing insurance. You are required to deliver to us promptly any documents served or delivered to You, Your agents or employees in connection with any claim or proceeding begun, or threatened, against You, Us or both parties.
Accident Reports. When a youth is injured/hurt in the program, an AF Form 1187 Youth Flight Accident Report will be filled out informing parents of the injury. Once the AF Form 1187 is filled out, parents will be notified by the front desk clerks, or managers in the program and ask to sign upon pick-up. If parents desire a copy of the report, a FOIA request will have to be made at the FOIA office by parents. Related to Accident Reports

  • Incident Report means a written report of any injury, accident, acts of physical aggression, or unusual incident involving an individual.

  • Safety Information means (a) any Adverse Event, including any Adverse Event related to a quality defect or received through a medical information inquiry, or (b) any Adverse Event related to a report of falsified or counterfeit medicinal product or (c) any of the following with or without an associated Adverse Event:

  • Accident/Accidental means an event or incident that occurs which is unforeseen and unexpected which resulted directly in the death or injury of the Person Covered.

  • Adverse Event means any untoward medical occurrence in a human clinical trial subject or in a patient who is administered a Licensed Product, whether or not having a causal relationship with such Licensed Product, including, without limitation, any unfavorable and unintended sign (including, without limitation, abnormal laboratory findings of clinical concern), symptom or disease temporally associated with the use of such Licensed Product.

  • Parent Reports has the meaning set forth in Section 4.5(a).
