This Investment Advisory Agreement (the “Agreement”), dated as of by and between Stawnychy Financial Services, Inc. (herein referred to as “Advisor”), and (herein referred to as “Client”), to provide Client with investment advisory services via its brokerage account established at the Client’s designated custodian as listed in Item 4 of this Agreement, herein referred to as the “Account”.
