This Release, Assumption of Risk, Waiver of Liability and Indemnification Agreement (this “Agreement”) is entered into by the undersigned in his or her own capacity (“Adult Participant”) and if any minor(s) is/are named in the signature block below (collectively, the “Minor Participant” whether one or multiple) on behalf of and as parent or legal guardian for such Minor Participant(s) (collectively, “Participant”), to and for the benet of the Released Parties (as defined herein) in favor of Winnwood Skate Center LLC (“Winnwood”). In consideration of Winnwood permitting Participant to enter into the roller-skating rink located at 4426 NE Winn Road, Kansas City, Missouri 64117 (the “Premises”) and to participate in any or all of the Activities (as defined below), Participant agrees as follows:

This Waiver of Liability and Indemnification Agreement (Agreement) is agreed to and executed by the Adult Participant (as identified by signature below) individually and on behalf of and as parent or legal guardian for any minor or incapacitated person, whether listed below or not, (Child Participant) using the facility and equipment located at 6334 Maple Avenue, Dallas, TX 75235 (Premises) in favor of Texas Pride Athletics, LLC (Texas Pride). Collectively and severally, Adult Participant and Child Participant are referred to as the Participant.

(hereinafter “Vision”) hereby agree that I hereby waive any and all claims which I have or might have or which might arise against Vision, related to my use of any of Vision’s resources, including, but not limited to its facilities, equipment and parking area. In addition, I hereby agree that I will 1 indemnify and hold Vision harmless from and against any and all losses, liabilities, claims, actions, damages and expenses, without limitation, howsoever they might arise, that may be incurred by Vision arising out of my use of the Club facilities, including but not limited to its facilities, equipment and parking area.
