This contract agreement is hereby made and entered into as of 1 July 2019 (the “Effective Date”), by and between the Florida Commission on Community Service, doing business as Volunteer Florida (the “Commission”), having its principle place of business at 1545 Raymond Diehl Road, Suite 250, Tallahassee, FL 32308, and, an entity organized under the laws of the State of Florida with its principal place of business located at 4499 Pine Forest Rd., Milton, FL 32583 (the “Provider”) (collectively, the “Parties”) and referred to herein as the “Contract Agreement.”
