THIS AGREEMENT, dated as of is entered into by and between Jane Smith Doe (hereinafter referred to as "Resident/Fellow Physician") and UPMC Medical Education, a Pennsylvania non-profit corporation (hereinafter referred to as "UPMC ME") which is the graduate medical education (GME) sponsoring institution and managing corporate entity for various UPMC system tax exempt hospitals operating Accreditation Council for Graduate Medical Education (ACGME) approved GME training programs as well as other healthcare discipline accredited graduate education programs.
