This Agreement is made and entered into between the University of Washington School of Dentistry (“Training Site”), located at 1959 NE Pacific Street, Seattle, Washington 98195 and XXX (“School”) located at XXX. The purpose of this Agreement is for Training Site, which is committed to training health care professionals, to provide desirable clinical learning experiences and facilities for dental student education.
