Development name is shown on a final subdivision and/or land development plan, prepared by plan preparer name, which plan, dated <date>, last revised <date>, was granted final subdivision and/or land development approval by municipality on <date>. The final subdivision and/or land development plan indicates that development name is located on street name. Development name contains approximately development acres, all within the zoning district.
