Science is a hands‐on laboratory class and safety in the science classroom is the #1 priority. To ensure a safe science classroom, a list of rules has been developed and these rules must be followed at all times. Two copies of the contract will be provided. One copy must be signed by both you and a parent or guardian before you can participate in the laboratory. The second copy is to be kept in your science notebook as a constant reminder of the safety rules.

This is to certify and verify that I, (student’s name), have been instructed in and understand each of the following safety rules and procedures of this science class. I agree to follow all of the safety rules set forth in this contract. I understand that if I do not follow all the rules and safety precautions, I will not be allowed to participate in science laboratory activities.
