registering patient (collectively referred to herein as “Member,” “you,” or “your”) will participate in the personalized care experience offered by Spry Healthcare, LLC, an Ohio limited liability company and affiliated entity of The MetroHealth System, with its offices located at 811 Prospect Avenue East, Suite 200, Cleveland, OH 44115 (“Spry”). This Agreement becomes effective on the date that your online enrollment, including acceptance of the terms and conditions of this Agreement, is complete (the “Effective Date”).
