This Greater Plains Collaborative Cooperative Medical Informatics Data Sharing and Network Infrastructure Agreement (the “Agreement”), effective as of the 1st day of December, 2014 (the “Effective Date”), is entered into by and among the following entities (each a “Party” and collectively the “Parties”):
