This Agreement (the “Agreement” or the “Contract”) is made and entered into by and between the New Mexico Human Services Department (“HSD”); the New Mexico Behavioral Health Purchasing Collaborative (the “Collaborative”); and, Presbyterian Health Plan, Inc. including any successors and/or assignees (“CONTRACTOR”); and is to be effective upon signatures by all parties.
