This agreement (“Agreement”) is made and entered into this 1st day of July, 2023, by and between SHC Services, Inc. d/b/a Supplemental Health Care, (hereinafter “SHC”), with principal offices located at 6955 Union Park Center Drive, Suite #400, Cottonwood Heights, UT 84047 and Unified School District #500, on behalf of itself, its schools, programs, and subsidiaries (hereinafter collectively referred to as the “Client”) with its principal offices located at 2010 N 59th Street, Kansas City, KS 66104. SHC and Client are sometimes referred to as a “party” or collectively as the “Parties”.

Redstone Center Drive, Suite 200, Park City, Utah 84098 and Pickerington Local School District, on behalf of itself, its schools, programs, and subsidiaries (hereinafter collectively referred to as the “Client”) with its principal offices located at 90 N. East Street, Pickerington, OH 43147. SHC and Client are sometimes referred to as a “party” or collectively as the “Parties”.
