In consideration of 3 STEP SPORTS LLC (including its subsidiaries, affiliates and ownership entities, “3STEP”) allowing the minor identified below (the “Minor”) to participate in any event operated by or sponsored by 3STEP, including, but not limited to, athletic competitions, games, tournaments, practices, camps, clinics, tryouts and related activities (each, an “Event”), I, on behalf of Minor, acknowledge, voluntarily accept and agree to the following:
