THIS QUALIFIED DENTAL PLAN ISSUER CONTRACT (“Agreement”) is entered into by and between the California Health Benefit Exchange, an independent entity established within the government of the State of California doing business as Covered California (the “Exchange”), and _, a dental plan issuer as defined in Title 10 California Code of Regulations (“CCR”) § 6410 (“Contractor”).
