This Premium Account Agreement (this Agreement) describes the terms of the arrangement between Blue Cross and Blue Shield of Massachusetts, Inc. and/or, for HMO Blue plans, Blue Cross and Blue Shield of Massachusetts HMO Blue, Inc. (together referred to as Blue Cross and Blue Shield) and the Account to provide health care benefits for the Account’s covered employees and their covered dependents (Members).
