THIS AGREEMENT FOR CHILD CARE SERVICES dated this _______________day of ____________________, 202__ between Valencia Randall-Walker DBA Little People Destined For Greatness Child Care LLC., Facility #198008514 (referred to in this contract as the “Provider”) is a family child care home licensed under the California Department of Social Services provide day care for up to 14 children. For this purpose, childcare means nonmedical care for children in need of personal services, supervision, age-appropriate activities and education. The provider accepts children from age 2 through 8 years old.
