this day of , 2017, is between Employee and Employer (referred to collectively throughout as “Parties”). The parties have agreed that Employee will separate her employment from Employer, as provided in this Agreement and, in connection with such separation of employment, Employer has agreed to provide Employee with certain benefits to which she would not otherwise be entitled absent the execution of this Agreement. In consideration of the mutual promises contained in this Agreement, Employee and Employer agree as follows:
