LIABILITY RELEASE: In consideration of participation in Archery Hunting at Crossway Church (330 Barbara Street, Millersville, PA 17551), I release from liability Crossway Church, its pastors, directors, employees, volunteers and teachers (collectively herein the “Church”) from all liability for accidental personal injury, sickness or death, as well as property damage and expenses, of any nature, which may be incurred by the Participant(s) while involved in the activities and child care. I, on behalf of my minor Participant(s), hereby assume all risk of accidental personal injury, sickness, death, damage and expense as a result of participation. I further agree to hold harmless and indemnify the Church for any liability sustained by the Church as the result of the negligent, willful or intentional acts of the Participant(s).
