*NOTE: Part XII. 11BPHARMACY MEDICATION BENEFITS is applicable only if the Subscribing Group elected, through the Master Application attached hereto, coverage of pharmacy medication benefits with their AvMed Large Group Empower Plan. If coverage of pharmacy medication benefits was not elected, Part XII does not apply, and coverage of pharmacy medications is explicitly excluded under this Contract.
