This EPICCARE LINK SITE LEVEL AGREEMENT (“Agreement”) is entered into byand between Hospital Sisters Health System on its own behalf and on behalf of all its affiliated hospitals and entities (“HSHS”) and (“Participant”) (each individually a “Party” and collectively the “Parties”). This Agreement shall beeffective as of the date on which the last of the Parties has executed this Agreement (“Effective Date”).
