By this agreement, I am authorizing the ADDA to act as my representative in accordance with Government Code § 6254.21(c)(3), and any other law, rule or regulation that permits the ADDA to seek removal of my personal information from Internet websites, to contact any third party Internet provider to request that my personal residence address and phone number be removed from publicly accessible Internet websites.
