This HIPAA Data Use Agreement (“Agreement”), effective as of _________________ (“Effective Date”), is entered into by and between the Board of Trustees of The University of Alabama for The University of Alabama at Birmingham (UAB), on behalf of the School of Medicine (“Recipient”) and (“Covered Entity”). The purpose of this Agreement is for Covered Entity to furnish Recipient with a Limited Data Set (“LDS”), as defined and, in accord with the HIPAA Regulations.
