THIS SERVICE AGREEMENT (this "Agreement") is made and entered into as of September 5, 2023 by and between Sumter County Board of County Commissioners ("CLIENT") with a principal place of business at 7375 Powell Rd., Wildwood, FL 34785 and HEALTHSOURCE SOLUTIONS, LLC ("HEALTHSOURCE") with a principal place of business at 3149 Fernbrook Lane, Plymouth MN 55447. Each party may be referred to in this Agreement as a “Party” or collectively as “Parties.”
