This Agreement (the “Agreement”) is by and between Health Care Providers Data Theft RPG, Inc. (“RPG”), a Delaware Corporation and Applicant. “Applicant” or “Member” shall mean the natural person indicated on the Application for Insurance & Membership (the “Application”) or, if an entity, the entity listed on the Application, its owners, directors, officers, employees, volunteers, and committee members, as well as any companies that are parents of, subsidiaries of, owned by, related to, or affiliated with Applicant.
