This University of North Carolina Hospitals (“UNC Hospitals”) Graduate Medical Education (“GME”) Appointment Agreement (the “Agreement”) sets forth the terms and conditions of the undersigned resident’s (the “Resident”) appointment to a training program that is: 1) approved by the Accreditation Council for Graduate Medical Education (“ACGME”) or Commission on Dental Accreditation (“CODA”), or is a residency program for individuals who have completed a Doctor of Philosophy, and 2) is sponsored or co-sponsored by UNC Hospitals.
