This Subscription Agreement constitutes the agreement ("Agreement") between Fayetteville Public Utilities ("we," "us" or "Fayetteville Public Utilities") and the user ("you," "user" or "Customer") of Fayetteville Public Utilities' residential and small business communications services and any related products or services ("Service"). This Agreement governs both the Service and any devices, such as a Multimedia Terminal Adapter, Analog Telephone Adapter or any other IP connection device ("Device" or "Equipment"), used in conjunction with the Service and it applies to all lines on each Fayetteville Public Utilities account. This Agreement shall be effective as of the date it is signed by Fayetteville Public Utilities (the “Effective Date”).
