Business Associate Addendum (“Addendum”), effective on the later of April 14, 2003 the Compliance Date (defined in Section 5.3 below) is entered into by and between Empire Health Care Solutions, and with an address at , on behalf of itself and its subsidiaries listed on Schedule A attached hereto (“Customer”) (each a “Party” and collectively the “Parties”).
