I, «FN» «MN» «LN», «Degree», accept the offer of a graduate medical education appointment as a resident physician in the Family Medicine Residency Program of Central Iowa Health System d/b/a Iowa Lutheran Hospital beginning June 18, 2018. I understand that the first six days of this agreement includes new resident orientation and the first day of my appointment as a resident physician is June 26, 2018.
