This Direct Care Patient Agreement (this “Agreement”) is entered into by and between Whitefish Wellness PA, a Texas professional association, doing business as Whitefish Wellness (the “PA”), and the undersigned patient (“Member”). The PA delivers supportive weight loss care via telehealth and in-person visits through Physician Assistants employed by the PA. This agreement is entered into by mutual voluntary consent.
