This agreement is between the student and the mentor focused on the 2020-2021 school year from August through April. After the student and the mentor have met to discuss the student’s professional development goals over the course of the year, the student should complete this form and return to Jonathan Scrimenti (jscrimenti@willamette.edu) no later than September 30, 2020.
