This Agreement is entered into effective as of January 1, 2023 (the “Effective Date”), between Personic Health Care LLC, with offices located at 905 W. Sproul Road, Springfield PA 19064 (“Personic”), and Delaware County acting through the Delaware County Health Department (DCHD) with offices located at 125 Chester Avenue, Yeadon, PA 19050 (“Partner”). Personic and Partner shall be referred to hereafter individually as a “Party” and collectively as the “Parties” to this Agreement.
