This form must be filled out, signed, and dated by all adult campers and visitors to Stoney Creek Ranch (“Stoney Creek” or “the camp”), or, if the camper or other visitor is a minor (under 18 years of age), by a parent or legal guardian of that minor camper. No one will be permitted to attend camp unless Stoney Creek Ranch staff have received this form completed, signed and dated. In this agreement, the camper and other visitors, including chaperones of visiting organizations are referred to as “campers.”

Medication Policy: Medications brought to camp MUST be given to the camper’s counselor to be handed out by our First Aid Staff. All prescription medications MUST be in the original container with the camper’s name, name of medication, and directions clearly marked on the pharmacy label. All over-the-counter medication must be in the original container and accompanied by parental instructions. Medication with no identification WILL NOT be given.

The Putnam County Fair Board hereby agree to allow your camper to be placed at the Eleanor Park Fairgrounds (Stock Barn Area) during the 2022 Putnam County Fair July 8-16, 2022 for a fee of $100.00. Camper owners understand that the Town of Eleanor does not operate a public camping facility in the Eleanor Park in Eleanor, WV. Owner also understands that such permission is granted to allow owners of 4-H /FFA livestock access to livestock care and to assist with any problems that may arise with the livestock during the fair. ALL CAMPERS WILL BE ASSIGNED SITES ON A FIRST COME/FIRST SERVE BASIS OR AS DESIGNATED BY THE CHAIRMAN. AGREEMENT MUST BE RECEIVED WITH FEE PRIOR TO ARRIVAL ON THE FAIRGROUNDS. PAYMENT MUST BE MADE BY MONEY ORDER OR CASH ONLY.
