Business Associate provides contract mental health services or is a community agency working collaboratively with Community Mental Health of Ottawa County (CMHOC). CMHOC is a member of the Lakeshore Regional Partners which is the prepaid health plan (PIHP) for Allegan, Kent, Lake, Mason, Muskegon, Oceana, and Ottawa Counties. As a member of the Lakeshore Regional Partners CMHOC and the Business Associate mutually agree to comply with the requirements within this addendum. Additional specifications regarding collaboration and/or services will be outlined in individualized contracts and/or designated agreements.
