Better Health Together (“ACH”) and _______ (“Participant”) entered into a Project Specific Agreement (“PSA”) effective July 1, 2020. ACH and Participant are amending the PSA to add Addendum 5.01 Pay-for-Performance. This Amendment will be effective on March 31, 2021.
