AHS Management Company, Inc. (“Ardent Health Services”) maintains the Ardent Health Services Welfare Benefit Plan (plan number 501), as such was amended from time to time (the “Plan”). It is now intended to further amend and to restate the Plan, and to provide for its continuation under the terms and conditions set forth herein, effective as of January 1, 2021 (the “Restatement Effective Date”), except as otherwise specifically provided herein.
