This amendment (the “Amendment”) is made and entered into by and between Lutheran Services Florida, Inc. d/b/a LSF Health Systems, LLC (“LSF”) and Alachua County Board of County Commissioners (“Provider”) to be effective April 15, 2020. Provider and LSF may be referred to herein individually as a “party” or collectively as “the parties.”
