This Master Lease Agreement, together with its definition exhibit, Exhibit “A”, attached hereto and made a part hereof by reference, is made on March 11, 2005, between APPLIED FINANCIAL OF PENNSYLVANIA, L.P., with its principal office at 6975 Union Park Center, Suite 200, Midvale, UT 84047 (the “Lessor”) and PENN TREATY NETWORK AMERICA INSURANCE COMPANY, a corporation organized in the state of Pennsylvania, with its chief executive office located at 3440 Lehigh Street, Allentown, PA 18103 (the “Lessee”).
