This Business Associate Agreement (“Agreement”) is entered into as of (“Effective Date”) by and between The Trustees of Columbia University in the City of New York, a New York not-for-profit corporation (“Covered Entity”) and (“Business Associate”). Each of Covered Entity and Business Associate may be referenced in this Agreement as a “Party” and collectively as the “Parties.”
