This Business Associate Agreement (the “Agreement”), is made effective as of the sign up date on the login information page of the CarePICS.com website, by and between CarePICS, LLC with an office at 6300 Westgate Road, Suite A, Raleigh, NC 27617 (the “Business Associate”) and the CarePICS, LLC Network Partner (the “Covered Entity”) with name and location(s) as entered in the Network Partner Registration page located at CarePICS.com. All information entered by Covered Entity located on the Network Partner Registration page of CarePICS.com is hereby incorporated into this Agreement by reference and made a part hereof.
