Written Requests. Memberships cannot be cancelled by phone or verbal conversation. Cancellation requests will only be honored if sent in writing to buyer’s assigned Crunch facility. All paperwork is deemed a request until staff can validate paperwork and approve such cancellation. A cancellation that has been denied will be sent in writing back to the buyer along with reason of denial or instructions for acceptance and refund. Buyer must keep a copy of their cancelation request form as proof of the request.

Written Requests. Loan Advances shall be made, at the Borrower’s written request to Administrative Agent, for the Revolving Facility, on the basis of written requests, made in accordance with the method and procedures described in Section 2.1.3 below; and Administrative Agent shall act upon such requests within three (3) Business Days following the receipt of a written request from Borrower for a LIBOR Rate Advance and within one (1) Business Day following the receipt of a written request from Borrower for a Base Rate Advance, which action may include, without limitation, funding the requested Loan Advance or specifying the basis for not funding and, when applicable, requesting additional information and supporting documentation. The date on which any Loan Advance is funded (or Letter of Credit is issued) is herein called a “Drawdown Date.”
Written Requests. Whenever practicable, nurses shall submit written requests for vacation at least four (4) weeks in advance of their vacation to their supervisor. No request may be submitted for a vacation period more than six (6) months in advance of this request. However, vacation requests of fourteen (14) consecutive days or longer may be submitted up to twelve (12) months in advance. In any Facility or Seniority Unit where other Bargaining Units have amended the period of time to submit vacation requests (more than six (6) months in advance), this section is hereby modified to conform with those amended periods of time. Supervisors shall respond in writing to all vacation requests within ten (10) calendar days after such request is made. Once the vacation has been approved, it shall not be rescinded by the employer except during an emergency. The nurse may rescind a vacation request with at least twenty-eight (28) days’ notice.
