When Benefits End Clause Samples
The "When Benefits End" clause defines the specific circumstances or events that will result in the termination of benefits provided under an agreement or policy. Typically, this clause outlines triggers such as the end of employment, reaching a certain age, or the occurrence of a disqualifying event like fraud or non-payment of premiums. By clearly stating when benefits cease, the clause ensures both parties understand the duration and limitations of coverage, thereby preventing misunderstandings and disputes regarding entitlement to benefits.
When Benefits End. The last payment will be made on the District's regular payroll date in June of the school year in which the retiree reaches age 65 or in the year in which the retiree is last eligible for benefits; or on the death of the retiree in which case the benefits will be pro-rated on a daily basis beginning with July 1 of the school year when the death occurs
When Benefits End. All of your benefits will end on the last day of the month your employment ends. Your employment ends when you cease Active Work as an Employee. However, for the purpose of benefits, the Employer may deem your employment to continue for certain absences. See CONDITIONS UNDER WHICH YOUR ACTIVE WORK IS DEEMED TO CONTINUE.
When Benefits End. The benefit ends on the earlier of: • The end of the maximum benefit period of 180 days • The date you become eligible for Long Term Disability • The date you are no longer disabled • The employee is offered and declines a temporary modified job in an employer provided Connecting to Work program • The employee is offered and declines a permanent modified or alternate job • The employee does not obtain employer approval for a leave of absence (or if the leave becomes unauthorized) • The employee does not comply with the recommendations of a case manager or independent medical examiner • The employee does not comply with any recommended appropriate care and treatment • The employee does not submit any required recertification • The employee is given official notification of termination of employment, or • This program is discontinued • The date employment is voluntarily or involuntarily terminated. An employee may appeal a decision to discontinue their benefit through the grievance process described in Article 14.
When Benefits End a. Employees who terminate their employment are eligible for reimbursement for services provided up to and including their termination date but not after. The deadline to request reimbursement is the last day of the month after your termination date.
b. A child ages out of eligibility the last day of the month in which they turn age 13, unless their birthday occurs on the 1st of the month; in which case eligibility ends the last day of the prior month. A child with a documented special need will continue to be eligible.
