Welcome Packet. Upon acceptance into the Guardian Special Needs Trust the Beneficiary or his or her legal representative will receive a receipt and Welcome Packet containing a copy of the fully executed Joinder Agreement for use in properly reporting the establishment of the Trust sub-account to the appropriate government agency(ies), as well as additional instructions for requesting distributions and blank Distribution Request Forms.
