A packaged agreement consisting of (if Applicable) a Standard Utility Adjustment Agreement along with the following attachments, Attachment “A” Plans, Specifications, and Estimated Cost, Attachment “B” Utility’s Schedule of Work and Estimated Date of Completion, Attachment “C” Eligibility Ratio, Attachment “D” Betterment Calculation and Estimates, Attachment “E” Proof of Property Interest, Quitclaim, Joint-Use Agreement and/or Permit, and Field Notes for quitclaim portion of easement.
