Urine Samples. Where urine or blood samples have been taken, the employee shall have the right to request split samples, with one half sent to the Employer's lab and the other kept frozen. (see 30.8) Specimen collection will occur in a medical setting and the procedure should not demean, embarass, or cause physical discomfort to the employee. Each step in the collecting and processing of the urine samples shall be documented to establish procedural integrity and the chain of evidence. The employee will complete a form indicating the use of all, drugs currently being taken and identify the prescribing Physician. The employee designated to give a sample must be positively identified prior to any samples being taken. Specimen samples shall be sealed, labeled and checked against the identity of the employee to ensure the results match the employee tested. Samples shall be stored in a secure and refrigerated atmosphere until tested or delivered to the testing lab representative.
