System Coordination. The HEARTH Act requires ESG recipients to consult with Continuums of Care in allocating funds for eligible activities; developing funding, policies, and performance standards for ESG activities; evaluating outcomes of ESG assisted projects; and development of procedures for the administration and operation of HMIS. The City works with the Heart of Illinois Homeless Continuum of Care (HOIHCOC) in order to coordinate the allocation, use and oversight of ESG funds. Subrecipients are subject to all HOIHCOC policies and procedures in place at the effective date of this agreement and the Subrecipient agrees to achieve compliance with any new policy or procedure implemented by the HOIHCOC throughout the duration of this agreement in a reasonable time frame as determined by the HOIHCOC and the City at the time of the establishment of new policy.
System Coordination. All Programs have systems for counting and reporting children with disabilities and for ensuring continuity and consistency in service delivery. Efforts may be coordinated by:
a. Developing a system so that time lines and persons responsible for reporting are clear.
b. Training staff regarding specific procedures, timelines and regulations for each agency.
c. Sharing existing interagency agreements that impact children with disabilities and their families.
d. Sharing information to ensure the uninterrupted delivery of services to children who are migrant and/or homeless.
System Coordination i. Coordinated Entry System (CES) The Coordinated Entry System (CES) functions throughout the San Diego region and connects individuals and families experiencing homelessness with the most appropriate and available housing options. Prioritization standards are determined by the Regional Task Force on the Homeless (RTFH). Information provided by homeless individuals and entered into the Homeless Management Information System (HMIS) is utilized by RTFH to triage homeless San Diegans into the most appropriate housing intervention. Contractor will participate in CES as established by RTFH and focus on:
1. CES standardized Vulnerability Assessment Tool in screening, referral, and admissions processes for all Program clients, when appropriate and as established by RTFH; and
2. Participation in housing navigation, case conferencing, or other integral components of CES, when appropriate and as established by RTFH.
ii. Youth Homelessness Demonstration Program Grant (YHDP)
1. To support the efforts of the RTFH and the YHDP Coordinated Community Plan, to provide more accessibility to mainstream programs for Transitional Age Youth (TAY) experiencing homelessness, Contractor staff will participate in trainings related to youth-specific service delivery, when and as requested by the City.
2. In accordance with the YHDP Coordinated Community Plan, Contractor will work with the City, RTFH, and the Youth Action Board (YAB) to incorporate suggested Program and service changes as applicable to ensure safe and stable environments for TAY.
System Coordination. Sunovion shall incorporate the necessary information from each of the Myovant Market Access Contracts, Myovant Government Contracts, Myovant Special Distributor Contracts, and Myovant GPO/IDN Contracts into the necessary Sunovion systems (and shall ensure that necessary information from each of the Sunovion contracts related to Products, including Sunovion Wholesaler Contracts, 3PL Contracts, and Sunovion GPO/IDN Contracts, are appropriately incorporated into the appropriate Sunovion systems) to facilitate performance of the RCP Services and GPR Services; and
System Coordination. A. The physical and financial interdependence of the Blue Plains Facilities requires a regular forum where technical and financial issues affecting more than one Party can be presented and discussed.
B. The existing committees, comprised of representatives of the Parties and formed to address Blue Plains Facilities issues, are hereby dissolved and replaced by a Blue Plains Regional Committee. The BPRC will be the sole coordinating body among the Parties for Blue Plains issues and will include one representative of each of the following: District of Columbia; Washington Suburban Sanitary Commission; ▇▇▇▇▇▇▇▇▇▇ County; Prince George’s County; and Fairfax County.
C. Each CAO will appoint one member and one alternate to the BPRC.
D. The BPRC shall meet monthly for as long as it is deemed appropriate, and shall meet at least quarterly thereafter.
E. The agenda of each meeting shall include, but not be limited to the following topics:
1. Interpretation of this Agreement;
2. Plant performance and compliance with permits, administrative orders and consent decrees;
