Survey Interval. For provider types subject to Medicaid waiver or Medicare certification, the Survey interval shall be based on Medicaid waiver and Medicare requirements. For provider types not subject to Medicare certification, the survey interval shall be as approved in the Medicaid State Plan or Waiver Agreement, but no greater than 36.9 months. DPHE shall prioritize scheduling of continuing Certification Surveys based on its review of complaints and previous Surveys.

Providers with past or present Deficiencies, which impact direct client care and/or which meet criteria identified in the Medicaid State Pl an or Waiver Agreement, and as agreed upon by the Department and DPHE for prioritizing Survey workload shall be surveyed sooner in the Certification cycle. Deficiency list. Upon completion of each Medicaid Provider Survey, DPHE shall prepare a written statement of Deficiencies identifying any standards the provider failed to meet. The written statement of Deficiencies shall be entered into the CMS Automated Survey Processing Environment (ASPEN) system. Provider plans of correction shall be made available to HCPF upon request in a format agreed upon by the agencies.
